It looked like it was going to end goalless until Chelsea were awarded a penalty kick in the 84th minute.
Scott McTominay fouled Armando Broja as the players jostled for position during a set-piece situation.
Jorginho stepped up to coolly convert the penalty past David de Gea.
A win would have flattered Chelsea as United had been the better side for much of the game.
Chelsea keeper Kepa Arrizabalaga had made five saves and was seconds away from recording a fifth clean sheet in a row.
Kepa thought he had produced his sixth save of the day deep into added time when his fingertips diverted a Casemiro header onto the post.
But the ball bounced back off the post before landing just millimeters over Kepa's goal-line.
The result left Chelsea fourth in the EPL table, one point ahead of United in fifth.
