The 24-year-old Argentine, who joined Girona on loan from MLS outfit New York City FC in July last year, netted twice in each half in Catalonia.
Vinicius Junior and Lucas Vazquez scored consolation goals for Real, who remain 11 points behind league leaders Barcelona.
Castellanos is the first player this century to score four goals against Real Madrid in a single La Liga game.
Girona have emerged as a bogey team for Real in recent years.
The two teams have now met six times in La Liga, with Girona winning three of those games. Their fixture at the Bernabeu earlier this season ended 1-1.
Castellanos has scored 12 goals in 31 appearances for Girona.
