The Patriots acquired defensive end Jerel Worthy, a former second-round pick, from the Packers on Tuesday in exchange for a conditional late-round draft pick, according to NFL.com's Chris Wesseling, citing Ian Rapoport. The move is pending a physical.

New England pulled off the move only minutes after it traded cornerback Justin Green to the Cowboys in exchange for defensive end Ben Bass.

The 24-year-old Worthy, who was the 51st pick of the 2012 draft out of Michigan State, hasn't practiced this summer due to a back injury. He missed all but 14 snaps last season "because he was slow to recover from a late 2012 ACL tear," Wesseling reports.

Worthy had 14 tackles, 2.5 sacks and one forced fumble in 14 games as a rookie.
